Official transcripts are provided directly from the College as requested by the student. Transcript request must be in writing and should include
your name, maiden name if applicable, address, phone number and social security number,
the complete address of where you want your record sent and your signature.
Click and print the Transcript Request Form, and supply the information for your
request. When finished, click your browser's "Back" button to return to this page. Mail or
fax your request to the attention of the Records Office @ 750 Chase Parkway, Waterbury,
CT. The fax number is 203-575-8085.
There is no charge for transcripts.
